PMHx default category trees

To ease the integration of this work inside FreeMedForms please format your proposition inside a <code> tag, like this:

"en:Main category";"fr:Catégorie principale";"de:..."...
  "en:Children are just indented with two spaces";"fr:Les sous-catégories sont juste décalées de 2 espaces"...
    "en:A sub-sub category";"fr:Une sous-sous catégorie"....
  "en:Another sub-category"...
"en:Another Main category";"fr:Une nouvelle catégorie principale"

That results in english as:

• Main category
  • Children are just indented with two spaces
    • A sub-sub category
  • Another sub-catgeory
• Another Main category

Or write directly using lists.

• Objectives: simple and efficient, not many catogories.

  • Risk factor
  • Medical
  • Surgical
  • Obstetric
• Family

• Objectives: adapated to cardiologists wishes
